Our credentialing process follows guidelines outlined by the Illinois Department of Health and The Joint Commission. In order to proceed with credentialing procedure, we require all of our physicians to provide documentation pertaining to his/her academic, residency, fellowship, employment, licensing, certification and liability-related data. All information provided to us will be primary source verified. The application should also detail specific privileges requested, including a listing of the procedures the physician will be performing and the type of anesthesia he/she will be personally administering (or supervising the administration of such). Even though our center has transfer agreement with a neighboring hospital, all applying physicians must also main privileges at one of the local hospitals. 

Below you will find a list of documents necessary to be credentialed at the 900 N Michigan Surgical Center. Once we receive the documentation, all information will be verified and approved by the center's governing body. Physicians should expect to be fully approved and cleared for performing surgeries at the facility in approximately 30 days after credentialing office receives full credentialing package.
